Statement of Compliance
1. I am thoroughly familiar with the contents of the program material to be taped and/or cablecast & agree that it will comply with all local, state and federal laws with regard to cable programming, and it will not violate any of the operating rules & procedures as a set forth by The Public Access Television Corporation. Also, it will not contain:-
| a. Advertising material design to promote the sale of commercial products or services, or materials which identifies any product, service or trademark or brand name in a manner which is not reasonably related to the non-commercial use of such a product, service, trademark or brand name on the program. |
| b. Commercial programming which in whole or in part depicts, demonstrates or discusses products, services or businesses with the intent or substantial effect of benefiting or enhancing a profit making enterprise. |
| c. The direct solicitation or appeals for funds or other things of value for any and all purposes. |
| d. Political endorsements or advertising by or on behalf of candidates for public office or for or against any proposition on an election ballot for referendum. |
| e. Material which is obscene. |
| f. Any programming which constitutes or promotes any lottery or gambling enterprise which is in violation of any local laws. |
| g. Materials which constitutes libel, slander, invasion of privacy or publicity rights, unfair competition, violation of trademark or copyright or which may violate any local or federal laws. |
| h. Material which has a reasonable probability of creating an immediate danger or damage to property or injury to persons or of creating a public nuisance. |
| i. Material which has a reasonable probability of causing a substantial obstruction of law enforcement or other governmental functions or services. |
2. I assume full responsibility for the content of all programming material cablecast and will ensure that such program material will not violate any right of 3rd party.
3. I have obtain or, before the program is cablecast, I will obtain all approvals, clearances, licenses, etc. for the use of any program material which I will cablecast, including but not limited to approvals by broadcast stations, network, sponsors, music licensing organizations, copyright owners, performers' representatives, all persons appearing in or refereed to in the program material and any other approvals that may be necessary to transmit program material over the Public Access Television Corporation cable channel in Great Neck-North Shore.
4. I indemnify and hold harmless the Public Access Television Corporation (PATC), its Directors, Officers and staff, against any claims arising out of any use of the program material that I cablecast or any breach of any claims in the nature of libel, slander, with application laws and unauthorized use of copyright material. I understand that I may be criminally or civilly liable for performing or producing such material which is cablecast.
5. I agree that I shall not represent myself or any other person involved in the community access cable casting or productions as an employee, representatives or agent of The Public Access Television Corporation or of Cablevision.
6. I agree to pay the cost of any repair or replacement of equipment or materials resulting from damage, misuse or theft while such equipment or materials are in my possession or control. I understand the penalties that apply if I do not return equipment or materials on time.
7. I shall not use the Public Access Television Corporation channel, equipment or facilities for any financial gain or other commercial purposes. I understand that programming produced with PATC equipment or facilities must appear on the Public Access Television Corporation channel.
8. I understand that false or misleading statements made in this application are grounds for forfeiture of the right to use The Public Access Television Corporation equipment and facilities. After acceptance by the Executive Director or Technical Coordinator, this application is non-transferable.
